Zusammenfassung
In unserer Klinik, der Zentralklinik Bad Berka, werden
pro Jahr ca. 30 komplexe Pankreasresektionen durchgeführt.
Davon erfolgen ca. 90 % aufgrund einer malignen
Erkrankung. Die Eingriffe reichen von Enukleationen
über klassische Pankreasteilresektionen bis hin
zu Pankreatektomien. Bei diesen Hochrisikoeingriffen
ist nicht nur operatives Können, sondern vor allem
das perioperative Management von entscheidender
Bedeutung. Trotz vieler Fortschritte in den letzten
Jahren auf diesem Gebiet, insbesondere auch durch
die anästhesiologische Mitbetreuung, ist die postoperative
Morbidität mit 40–50 % noch immer sehr hoch.
Die Mortalität sank in den letzten Jahren auf ca. 5 %.
Das Ziel dieser Arbeit sind die Erarbeitung eines
anthroposophischen Verständnisses des Pankreasorgans, die perioperativen Besonderheiten der
Patient:innen mit Pankreaskarzinom und die daraus
erfolgende Ableitung eines perioperativen anthroposophischen
Handlungsrahmens.
Abstract
In our clinic, Zentralklinik Bad Berka, about 30 complex
pancreatic resections are performed per year. About
90 % of these are performed due to malignant disease.
The interventions range from enucleations to classic
partial pancreas resections to pancreatectomies. In
these high-risk interventions, not only surgical skill
but above all perioperative management is of decisive
importance. Despite much progress in this field in
recent years, especially through anaesthesiological
co-management, postoperative morbidity is still very
high at 40–50%. Mortality has fallen to about 5% in
recent years.
The aim of this work is to develop an anthroposophical
understanding of the pancreas organ, the perioperative
particularities of patients with pancreatic carcinoma
and to derive from this a perioperative anthroposophic
framework for action.
